Why Altitude Matters More Than Fitness

Kilimanjaro's summit sits at 5,895 metres, where the air holds roughly half the oxygen available at sea level. Your body needs time to adjust to that drop, and the biggest single cause of failed summit attempts is not exhaustion or poor fitness — it is altitude sickness brought on by climbing too fast for the body to keep up.

This matters because altitude sickness does not follow the rules you might expect. Marathon runners have turned back with severe symptoms while first-time hikers have summited comfortably. Genetics and ascent speed decide most of the outcome, which is exactly why route choice and pacing matter more on Kilimanjaro than on almost any other trek of its kind.

Symptoms: What Altitude Sickness Feels Like

Altitude sickness exists on a spectrum. Most climbers experience some mild symptoms at altitude, and mild symptoms are not a reason for alarm on their own. What matters is recognising the difference between manageable discomfort and warning signs that require immediate action.

The golden rule of altitude sickness: never ascend with worsening symptoms. Mild symptoms that are improving or holding steady can usually be managed in place. Symptoms that are getting worse, especially a headache that does not respond to painkillers or any loss of coordination, mean the safest direction is down, not up.

Understanding HACE and HAPE

Beyond standard AMS, two rare but serious conditions can develop at altitude. Both are medical emergencies, and both are why experienced guides and daily monitoring matter so much on a climb like Kilimanjaro.

HACE develops when fluid builds up in the brain due to the effects of low oxygen. It is rare but can progress quickly from confusion to loss of consciousness if untreated. It usually develops from worsening AMS rather than appearing without warning.

  • Watch for: confusion, severe headache, loss of coordination, difficulty walking a straight line
  • Simple field test: ability to walk heel-to-toe in a straight line, checked by guides when AMS is suspected
  • Treatment: immediate descent, the only effective response once symptoms appear

HAPE occurs when fluid builds up in the lungs, making it harder to breathe even at rest. It can develop alongside or separately from HACE and is also a medical emergency requiring immediate descent.

  • Watch for: breathlessness at rest, persistent cough, gurgling or crackling sound in the chest, extreme fatigue
  • Often noticed: at night, when breathing difficulty becomes more obvious lying down

How Your Route Affects Your Risk

Time is the single most effective tool against altitude sickness. Routes that spend more days on the mountain, and that follow a "climb high, sleep low" pattern, give your body far more opportunity to adjust before you reach the summit.

If you are choosing between routes and altitude sickness is a concern, the extra one or two days on Lemosho or the Northern Circuit is the most effective investment you can make. How to Reduce Your Risk

You cannot eliminate the risk of altitude sickness entirely, but several proven habits meaningfully reduce it. None of these require special fitness — they are about pacing and awareness.

Before You Travel

  • Choose a longer route: Lemosho or the Northern Circuit over Marangu
  • Spend a weekend at 3,000 metres or higher in the months before, if you can
  • Discuss Diamox with your doctor at least six to eight weeks ahead
  • Arrive well-rested — sleep debt makes acclimatization harder
  • Avoid alcohol in the days immediately before your climb

On the Mountain

  • Walk slowly — "pole pole" is not a suggestion, it is the strategy
  • Drink 3–4 litres of water daily, even without feeling thirsty
  • Eat on schedule, even when appetite drops (which is normal)
  • Report any symptoms to your guide immediately, however mild
  • Avoid sleeping pills, which can mask breathing changes at altitude

Diamox (Acetazolamide)

Diamox is a prescription medication that speeds up acclimatization by encouraging deeper breathing. Many experienced Kilimanjaro climbers take it as a precaution; others climb without it and adjust fine. We do not prescribe medication, but we strongly recommend discussing it with your doctor or a travel medicine clinic well before your trip, so you understand any side effects and how your body responds before summit night.

How We Monitor Every Climber

Altitude safety on Kilimanjaro depends as much on your guide's training as it does on your own preparation. Here is exactly what happens on every Refresh Africa Tours climb.

  • Pulse oximeter checks, twice daily. Every climber's blood oxygen saturation and heart rate are measured and logged each morning and evening.
  • Trained guides at every level. Our lead guides are trained to recognise AMS, HACE, and HAPE, and to act immediately rather than wait for symptoms to worsen.
  • Emergency oxygen on every climb. Supplemental oxygen is carried on all our Kilimanjaro itineraries as standard, not as a paid extra.
  • Descent protocol. If symptoms progress beyond mild, descent begins without delay — guides do not wait for permission or try to push a climber through worsening symptoms.
  • Daily briefings. Guides check in with each climber individually, not just as a group, since symptoms are easy to underplay in front of others.

Speak up early. The most common mistake we see is climbers downplaying symptoms because they do not want to slow the group or risk being sent down. Reporting a mild headache on day two is how it stays mild. Our guides would always rather know too early than too late.

Diamox, or acetazolamide, is a prescription medication many climbers use to speed up acclimatization. It is not required for everyone and it is not a substitute for climbing slowly.
